TOURIST / VISIT FRIENDS OR DISTANT RELATIVES

REQUIREMENTS :

1. PHILIPPINE PASSPORT
2. VISA APPLICATION FORM
3. PHOTO (4.5cm x 4.5cm with white background.)
4. BIRTH CERTIFICATE OF APPLICANT (NSO issued with receipt and valid within (1) year.)
5. MARRIAGE CONTRACT (If Married Applicant) NSO issued and valid within (1) year.
     • In case of non-record, you must submit certificate of non-record together with the one from Local Civil Registrar.
     • Exemption of Birth Certificates and Marriage Contract
      -If the applicant is has his/her passport (old or valid) with a used Japanese Visa.

7. DOCUMENTS OR PHOTOS TO PROVE RELATIONSHIP ( If visiting a friend)
8. INVITATION LETTER FROM GUARANTOR IN JAPAN ( not applicable for package tour)
9. BANK BALANCE CERTIFICATION OR A COPY OF BANK BOOK PAGES FOR THE PAST 2 YEARS.
   (Required if applicant is SHOULDERING ALL TRAVEL EXPENSES.)
10. PHIL. INCOME TAX RETURN (Form 2316) Original and (1) photocopy.
    (Required if applicant is SHOULDERING ALL TRAVEL EXPENSES.)
      • 9 & 10 are only applicable if he/her supporter has deposit in bank or income tax return.
      • Exemption of 9 & 10
      (If the applicant is eligible for Multiple Entry )
11. RESIDENCE CERTIFICATE (JUMINHYO)
      • If guarantor is a Japanese.
12. FOREIGN REGISTRATION CERTIFICATE FROM CITY HALL
      • If guarantor is not a Japanese
      • If guarantor in Japan shoulders any part of travel / living expenses
      ** IF GUARANTOR IN JAPAN WOULD BE THE ONE SHOULDERING TRAVEL & LIVING EXPENSE **
13. GUARANTEE LETTER (Mimoto Hoshosho) format available at the Japan Embassy Website or at the entrance of Embassy.
14. INCOME CERTIFICATE FROM CITY HALL (Shotoku Shomeisho) or Tax Return Certificate from Tax Office ( Nouzeishomeisho, form 2) or Kakuteinshinkokusho Hikae or BANK CERTIFICATE (Yokin Zandaka Shomeisho).
      • “Gensen-Choshu-Hyo” is not accepted.

PLEASE NOTE : It is applicant's responsibility to ensure that he/she meet the requirements for the grant of a visa. Submission of any supporting documents not listed above is encouraged (e.g. applicant's economic or social ties with the Philippines, urgent reasons for visit : medical certificate, wedding invitation.)

Submission of requirements does not guarantee issuance of visa.
Validity of Documents is three (3) months from the date of issue
(unless otherwise stated ).

ADDITIONAL DOCUMENTS :

1. If Birth Certificate from NSO is unreadable.
2. If there is no record in NSO, (in addition to "Certificate of Non-Record" from NSO Headquarter)
3. SUBMIT: Birth certificate from Local Civil Registrar
4. If Birth Certificate (NSO) is "Late Registration"
5. Baptismal Certificate
6. School Record (From 137)
7. School Yearbook (If applicable)
8. Please indicate complete address and landline number (if applicable) of the church in the baptismal certificate, and of the school in school record.

FILING APPLICATION :

1. Application should be filed through accredited agencies.(Please see our website for the list of Agents or get one from FAX service by Ministry of Foreign Affairs in Tokyo, or get one from visa gate of the Embassy.)
2. The applicant may be required to appear in person for interview etc.

NOTICE :

• Applicants are given three (3) months to submit additional requirements. Otherwise application will be terminated.
• Documents submitted for application will not be returned.
• Applicants can get information from accredited agencies when his/her application is pending.
• When visa is granted, VISA FEE is gratis for this category.(Philippine passport holder)
• When visa is not granted, the reasons of denial will not be disclosed. Applicants may apply again after six (6) months with the same purpose, unless there are emergency / humanitarian reasons or application for different purpose.
