VISIT RELATIVES (PARENTS, SISTERS/BROTHERS, AUNTS/UNCLE OR NIECE/NEPHEWS)

REQUIREMENTS:

  1. PHILIPPINE PASSPORT
  2. VISA APPLICATION FORM
  3. PHOTO (4.5cm x 4.5cm with white background.)
  4. BIRTH CERTIFICATES TO PROVE REALATIONSHIP
  5. MARRIAGE CONTRACT (If Married Applicant) #4 & 5 must be NSO issued and valid within (1) year.
  6. INVITATION LETTER ( must have a handwritten signature)
  7. FAMILY REGISTRATION ( Koseki Tohon)
  8. RESIDENCE CERTIFICATE (Juminhyo)
      • 7 & 8 only applicable if guarantor is a Japanese.
      •
    NOTE: 7 & 8 must be issued within three months.
  9. FOREIGN REGISTRATION CERTIFICATE FROM CITYHALL, PASSPORT COPY ( all pages) or copy of FOREIGN REGISTRATION CERTIFICATE CARD (is required if relative in Japan is not a Japanese).
       ** IF APPLICANT IS SHOULDERING ALL TRAVEL & LIVING EXPENSES **
  10. BANK BALANCE CERTIFICATION OR A COPY OF BANK BOOK PAGES FOR THE PAST 2 YEARS.
       ( Required if applicant is SHOULDERING ALL TRAVEL EXPENSES. )
  11. PHIL. INCOME TAX RETURN (Form 2316) Original and (1) photocopy.
      (Required if applicant is SHOULDERING ALL TRAVEL EXPENSES.)
       *** IF GUARANTOR IN JAPAN WOULD BE THE ONE SHOULDERING TRAVEL & LIVING EXPENSE:
  12. GUARANTEE LETTER (Mimoto Hoshosyo) format available at the Japan Embassy Website or at the entrance of the embassy.
  13. INCOME CERTIFICATE FROM CITY HALL (Shotoku Shomeisho) or TAX RETURN CERTIFICATE FROM TAX OFFICE ( Nouzeishomeisho, form 2) or Kakuteinshinkokusho Hikae or BANK CERTIFICATE (Yokin Zandaka Shomeisho).
      
    EXEMPTION OF 13:
       • If the applicant is a parent of Guarantor in Japan
       • If the applicant is a child ( must be 19 years old and below ) of guarantor in Japan.

PLEASE NOTE: It is applicant's responsibility to ensure that he/she meet the requirements for the grant of a visa. Submission of any supporting documents not listed above is encouraged (e.g. applicant's economic or social ties with the Philippines, urgent reasons for visit : medical certificate, wedding invitation.)


Submission of requirements does not guarantee issuance of visa.
Validity of Documents is three (3) months from the date of issue
(unless otherwise stated ).


ADDITIONAL DOCUMENTS :

  1. If Birth Certificate from NSO is unreadable.
  2. If there is no record in NSO, (in addition to "Certificate of Non-Record" from NSO Headquarter)
  3. SUBMIT: Birth certificate from Local Civil Registrar
  4. If Birth Certificate (NSO) is "Late Registration"
  5. Baptismal Certificate
  6. School Record (From 137)
  7. School Yearbook (If applicable)
  8. Please indicate complete address and landline number (if applicable) of the church in the baptismal certificate, and of the school in school record

FILING APPLICATION :

Application should be filed through accredited agencies (Please see our website for the list of Agents or get one from FAX service by Ministry of Foreign Affairs in Tokyo, or get one from visa gate of the Embassy.)
The applicant may be required to appear in person for interview etc.


NOTICE :

  1. Additional documents may be requested where necessary. Applicants are given three (3) months to submit additional requirements. Otherwise application will be terminated.
  2. Documents submitted for application will not be returned.
  3. Applicants can get information from accredited agencies when his/her application is pending.
  4. When visa is granted, VISA FEE is gratis for this category.(Philippine Passport Holder)
  5. When visa is not granted, the reasons of denial will not be disclosed. Applicants may apply again after six (6) months with the same purpose, unless there are emergency / humanitarian reasons or application for different purpose.
